- Select on the right side of the login page.

How to Enroll in Online Access
- Visit the home page, on that select ‘register’ link at the right side of the page.
- Select your state and select the type of insurance you want, enter your policy number, member number, mailing address, and zip code.
- Enter the personal details of the policyholder and read the details and confirm the details.
- Then click the ‘Submit’ button.
How to Get a Quote
- In the home page of ALFA Health Insurance, select the ‘Get a Quote’ form on the right side of the screen.
- Select ‘Health’ from the drop-down menu and enter your zip code. Click the ‘Submit’ button.
- Enter your personal details like the first name, last name, email address, phone number, date of birth, country and select the state coverage.
- Click the ‘Quote’ Button to process.
How to Make a Claim
- Go to the home page, on that select ‘Claims’.
- On the drop-down menu, select ‘Report a Claim’.
- Select the ‘Health’ type, enter your policy number and state and click ‘Next’.
